The O’Connell House is seeking an enthusiastic, driven, and dynamic Case Manager to provide and implement superior clinical care and clinical programming to our clients. The Case Manager is the supervisor entrusted with the execution of mental health care program services to give our clients the best chance to succeed and thrive after treatment. Job Description: The Case Manager is responsible to for the overall operations, development and supervision of the mental health care program (MHCP) services. In collaboration with the Program Director and TOH Clinician, the Case Manager will assist in creating and implementing programming, ensuring programming provides best-care practices, provides trauma-informed care, and meets the state and federal regulatory standards. The Case Manager will supervise and provide training to staff, and volunteers. The Case Manager must possess extensive knowledge of trauma-informed practices and care, strong ability to solve complex problems, and have excellent decision-making skills, including critical thinking. The Case Manager must also be mission oriented. In addition to implementing and overseeing the MHCP services, the Case Manager will also provide and/or coordinate individual and group therapy to survivors of sex trafficking. The Case Manager will collaborate with other therapeutic clinicians and providers and other therapeutic services to make sure residents are receiving proper and specific care that will benefit them. The Case Manager will take all direction from the TOH Clinician, who answers to the Program Director

The Case Manager will possess skills and knowledge of operations of a clinical team. The Case Manager will be familiar with a variety of therapeutic modalities, theories and practices and have a thorough understanding of Evidence-Based Treatment practices. This individual will be familiar with best practices for relevant levels of care and environments. The Case Manager will possess managerial and supervisory skills and utilize these skills on a daily basis.
Qualifications – Bachelor degree minimum, Master’s Level preferred, State and/or Board Licensed Clinical Psychologist, Social Worker, LMHC, or APRN/RN with experience in mental health treatment environment and OBGyn /midwifery a plus! Managerial experience with treatment of co-occurring disorders, trauma disorders and experience with homelessness, domestic violence and/or survivors of sex-trafficking. Knowledgeable in regulatory compliance with state board standards of care.
- Preferably three to five years of experience working in relevant field.
- Excellent conflict resolution, crisis intervention, conflict resolution, de-escalation, and staff training skills.
- Technologically skilled with demonstrated proficiency in Electronic Health Records, and Microsoft Office.
- Working clinical knowledge of criteria for admission, intake, assessment, continued stay, and discharge/termination.
- Work in a faced-paced environment; make independent decisions, communicate effectively when dealing with clients, families, staff, and other stakeholders.
- Refined and well-organized multi-tasking skills and in-depth problem-solving abilities.
- Current First Aid and CPR certification.
Responsibilities and Duties:
- Uphold and enforce The O’Connell House’s (TOH) standards, policies and procedures, client rights, and professional code of ethics and conduct, and complete additional duties as required if deemed necessary by administration.
- In collaboration with the TOH Clinician, create an intake process for survivors of sex trafficking to assess appropriate referrals for the program
- In collaboration with TOH Clinician, screen referrals and conduct intakes with Residential Team
- Conduct initial assessment within 24-48 hours of new resident at the O’Connell House and create individualized mental health treatment plan
- Provide individual counseling for survivors of sex trafficking, as needed
- Provide diagnosis, case conceptualization, and treatment planning of resident with the TOH Clinician.
- Coordinate with the referring agencies, community contacts, family, legal system, and other professionals involved in the resident’s treatment.
- Evaluate and update individualized treatment plan as needed
- Maintains timely and quality documentation of progress notes, admission paperwork, and monthly reports.
- Provide group counseling to residents
- In collaboration with TOH Clinician, verify that programming and implementation of programming is trauma-informed and appropriate for clients’ needs
- Train staff, religious sisters, and volunteers in working with this population
- Develop appropriate training program in compliance with policies and procedures
- In collaboration with the TOH Clinician, create policies and procedures.
- Complies with rules and regulations regarding ongoing review of clinical caseload to meet all licensing, state and federal requirements
- Participate in weekly staff meetings
- Maintain client files according to licensing requirements
- Assists in transitions to next level of care by coordinating with other agency and/or family members, transitional housing, and all other professionals involved in the planned departure from the O’Connell House.
- Consults with the residential team weekly regarding clinical issues and helps direct the delivery of trauma-informed interventions with individual clients.
- Provides crisis management on an as needed basis to support direct care staff.
- Complete any other responsibilities or duties assigned by the TOH Clinician to support individualized treatment planning and case-management needs within the program
- Oversees art and music healing therapy
- Responsible for continuous quality management and performance improvement.
- Will uphold and respect all Catholic teachings and be respectful of the Religious Sisters.
